  2. Complete Your Intake Forms

    We’ll send you a secure link to complete health history forms before your visit. No clipboard paperwork on arrival.

  3. Meet Your Physician

    Your first visit is 45 minutes — not the standard 15. We take time to understand your history, concerns, and health goals.

  4. Build Your Care Plan

    We’ll create a personalized preventive care roadmap, order any necessary labs, and coordinate specialist referrals if needed.
