What We Provide

Six core services. One interdisciplinary team. Coordinated wherever you call home.

Hospice Care

Full-spectrum end-of-life care including physician oversight, skilled nursing, pain management, medical equipment, and medications related to the terminal diagnosis. Provided wherever you call home.

Palliative Care

Comfort-focused care for patients with serious illness who are not yet on hospice. We work alongside your existing medical team to manage symptoms and improve quality of life.

Respite Care

Short-term inpatient care (up to 5 days) at a Medicare-certified facility. Gives family caregivers a needed rest while we continue full hospice services for your loved one.

Continuous Care

During periods of crisis, we provide up to 24 hours of continuous skilled nursing in the home to manage acute symptoms until comfort is restored.

Veteran Services

Specialized programs for veteran patients including We Honor Veterans partnerships, pinning ceremonies, and staff trained in military culture and veteran-specific end-of-life issues.

Bereavement Support

Grief counseling for 13 months following a loss. Individual therapy, support groups, children's grief programs, and annual memorial services to honor those we have cared for.

Getting Started with Hospice

Four unhurried steps. We move at your family's pace, never faster.

  1. Initial Consultation

    Call us or have your physician make a referral. A nurse or social worker will visit within 24 hours to explain services and answer questions.

  2. Eligibility Assessment

    Our medical director reviews the patient's condition to determine hospice eligibility. Most patients with a prognosis of six months or less qualify.

  3. Care Plan Development

    We create an individualized care plan with the patient, family, and attending physician. This covers medical, emotional, spiritual, and practical needs.

  4. Ongoing Care & Support

    Regular nurse visits, on-call support 24/7, and the full interdisciplinary team. We adjust care as needs change and are always just a phone call away.
